Vermont Annual Statement Filing Instructions for Domestic Risk Retention Groups Associations and SPFC Captive Insurance Companies

3. License Fee Renewal Make check payable to "VT Dept of Banking and Insurance" or "VT BISHCA"
License renewal fee = $500
8 VSA, Chapter 141, Section 6002
4. Premium Tax Payments

Note:  A copy of the Premium Tax Return should be included with the annual statement filing.

5. Delivery Instructions:  Note 8 VSA, Chapter 141, Section 6007 states that annual reports are due "prior to March 1". All filings must be received (or postmarked) no later than the indicated due date.  If the due date falls on a weekend or holiday, then the deadline is extended to the next business day.  PLEASE PLACE ALL SUBMITTED MATERIAL INSIDE THE COVER PAGE OF YOUR ANNUAL STATEMENT BOOKLET.
6. Late Filings Must be approved by the Deputy Commissioner of Captive Insurance. 
7. Signatures  Quarterly and Annual Statements:  At least two original signatures of executive officers are required on the quarterly and annual statements.  Signatures should be notarized.

Reinsurance Attestation Supplement:  This supplement requires the signatures of the "Chief Executive Officer" and "Chief Financial Officer".  Vermont will accept the signatures of their functional equivalents, i.e. President and Treasurer.

RBC Report and other Supplements:  As the results of the RBC Report are incorporated within the 5 year historical data of the Annual Statement, signatures are not required on the RBC Report.  Other Supplements do not require signatures.
8. Electronic Filings  The Captive Insurance Division does not accept electronic filings.  All filings should be hard copy format.
