The Insurance Consumer Section is dedicated to helping Vermonters with insurance questions and complaints for all types of insurance.

There are links on the left hand side of this page where you can obtain information about how to file an insurance complaint; obtain an answer to an insurance question; review consumer advisories and obtain a brochure.

Filing A Complaint

To file a complaint with the Insurance Division, please complete the Insurance Complaint form.  You can call and request a complaint form by email or U.S. mail or you can download a complaint from this site.  You or your authorized representative must sign the complaint form in order for it to be reviewed.  The complaint form should provide us with the details of your complaint, including how you have tried to resolve the matter and copies of supporting documents.  Be sure to include what you are seeking as a fair resolution.

If your complaint is in reference to health insurance (this includes HMO, PPO, indemnity, Catamount Health, disability income, Medicare supplement, long-term care, accident and other disease-specific insurance like coverage for cancer, student insurance, and dental insurance), we encourage you to call Consumer Services at 1-800-964-1784 to discuss your situation. We can help you determine whether filing a formal complaint to the Department is the best way to address the matter, or we can evaluate whether you may qualify for an Independent External Review.

Appealing a Denial

If you have had medical services which have been denied and you think they should be covered, you must complete the Health plan's internal process, as described in your notice of decision letter. If you are still unhappy with your plan’s decision, you may have the right to get an independent review of that decision by appealing your denial. Contact Consumer Services at 1-800-964-1784 to see if you qualify for external appeal.

If it is determined  that you qualify for an appeal, you can download an external appeal form.
